Forum VB4 Duplicate meta descriptions problem in webmaster tools

Hi Guys, I have thousands of Duplicate title tags and Duplicate meta descriptions when i check in google webmaster tools.
How can i resolve this for vb 4.0.5?

Meta descriptions look like this for the same post
