Forum Paste As Plain Text Icon Now Showing on Custom Style

On my custom style, I am having issue with the Paste As Plain Text icon not showing properly on the Editor. On the default style it shows but on the custom style it looks like the image that I've attached.

Things that I have tried:

Checking editor.css, editor-ie.css, and editor_contents.css to see if they were customized. None of them were modified from stock and therefore were ineligible to be reverted.

Uploading the clientscript/ckeditor directory from default vB install package.

Uploading the images/editor directory (with sprite.png) from the default vB install package to my /images/customtheme/editor directory.

Double checking the editor directory's path in the imgdir_editor style var.

Disabling all plugins via config.php.

Not really sure what else I can do. Could anyone help please?
