Forum Custom Avatars, Profile Pictures, and PM problems - need assistance

AdminCP 4.2.0 Patch Level 2 - Licensed (I'm not the owner of the forum)

I'm one of our forums Administrators.

Beginning within the past 48 hours (not sure exactly) our forum members (except for our Administrators and one single Retired Administrator) no longer have their custom avatars or profile pictures displayed, nor can the Administrators see them outside of the AdminCP > User > Image Options, where they are displayed. Our avatars and profile pictures are stored in the database.

Also, our members no longer have permissions to make changes from within their Settings or to PM anyone. They can receive PMs, but can't reply to them. See the following comment from one of my members received today:

As for the PMs, I notice I have a Notification and I select to view it. Once there, I find no way to reply to you. The only option I have is to "Delete this Message". I also have no option to send you a message by clicking on your name (only options: View Profile, View Forum Posts, View Blog Entries, and View Articles). When I try to click "Send New Message" I get another access denied message (same as above).

I don't know what happened that has caused these issues. I could really use some troubleshooting experts to help me figure out what happened, and more importantly, what to do to fix the issues.
