Forum Upgrade Problems 3.8.4 to 4.0.1: fetch_template / Includes/ errors?? Please help.

Earlier today I upgraded my 3.8.4 forums to 4.0.1. I have reverted all of the old templates that didn't work and yet I keep getting errors. Some of them include the following:

Warning: fetch_template() calls should be replaced by the vB_Template class. Template name: welcome_headers in [path]/includes/functions.php on line 3893

Warning: Cannot modify header information - headers already sent by (output started at [path]/includes/class_core.php:4501) in [path]/includes/functions.php on line 3742


Can anyone clue me in on what I did wrong. I followed the instructions in the read me file to a T.
