Forum Users cannot upload attachments to the board

After upgrading to 4.0.1, users are unable to upload attachments to the board. This appears to be some sort of javascript issue, but I haven't had any luck tracking down the problem.

User can click the manage attachments box, and can then click the Add Files button and can then click the Select Files button. However, even after selecting a file in the dialog box, the upload button is never shown. Additionally, if the user then adds a second attachment, it is not shown in the list, but the upload size text changes.

It's a very frustrating issue.
