/forum/images/attach icons missing

One of my attach icons (kmz.gif) is not showing up. My process was:

• Place a 16x16 GIF89A kmz.gif in the /forum/images/attach subdirectory
• Create a KMZ entry using Admincp | Attachments | Attachment Manager

Yet, when attempting to attach a KMZ file to a new post, the KMZ icon doesn't show.

Any thoughts what I may be doing wrong?

(Haven't upgrades to 3.8.4 yet - Still running 3.8.3)
