cannot upload images - security token error

This is the error received:

Your submission could not be processed because a security token was missing.

If this occurred unexpectedly, please inform the administrator and describe the action you performed before you received this error.

I ran a test in diagnostics. These are results:

Results
No file uploaded and no local file found. The test cannot continue.
