Forum Missing Add Files Button
 

I've found that I can't upload any pictures to my albums due to missing the Add Files button.

I'd found a ticket that seemed to address this and was supposedly fixed in 4.0.2 but I'm still not seeing on my live board (v4.0.3 PL1) or my dev site (v4.0.6)

I did find on another site my attachment manager popup is missing the DIV tag where the Add Files button lives, however I'm having trouble locating the template of the popup.
