Users getting put into thread moderation..
 

I'm having this issue where a few users were randomly put into moderation when they post a thread. I've searched through user group options, admin options, and user options to see where they could possibly have been put into moderation when posting a thread and I don't see anything wrong. Every option is how it should be. Is there a fix for this? Can someone please help?
