CMS Image Attachment preview issue

We are having a problem with the attachment system. When you create a new article and attach an image it seems to work fine. Yet when the article is posted the image breaks. I tried checking all the attachment tables for problems but they are fine.

Our site was upgraded from VB3 recently. Not sure if this broke anything, but the attachment system has never worked in vb4.

Any help?
