Append to URL
 

I want to create a input=text and what is typed into that, I want to basically pass to a URL to be loaded.

So for example, in the text box I enter:
testing123

When I hit go I want to load a url with that added to it:
domain.com/blah.php?do=search&query=testing123

Any idears?
