Forum Pictures randomly shows up like picture, link, link and pic in "Attached Thumbnails".

All pictures have been uploaded in the same way: "insert image" and uploaded from computer. Some shows up like they should, some just like a link. At the end of the post some appear in "Attached Thumbnails". they randomly shows up like link and picture.

What do I have to do to get all the pictures show up like they should (thumbnail picture, none should show up in the "Attached Thumbnails" box)?
