Forum Database error when installing mod
 

I just upgraded to 4.0.3 from 3.8.4 on my test site. Everything went smooth. All my posts/users are there. I'm having trouble when trying to install mods though. I successfully installed a chatbox http://www.vbulletin.org/forum/showthread.php?t=235271

That chatbox installed and works flawlessly. The problem is when I try to install any other mods.

and a few others I keep getting this error:
