CMS Javascript Form in Widget Works - But Javascript Code Also Shows
 

I'm trying to embed a javascript form into a static html widget. The form actually works when the widget is displayed, but a lot of the javascript code is also shown. Does anyone know what is wrong and how to clean this up?
