New user somehow slipped past moderation
 

I have set "registered users" group to require moderation, so all new members have their posts moderated. Once I know they are kosher I move them to an approved group.

This usually works fine, but this morning, a newly registered user, still in the "registered users" group, posted and the post appeared on the forum without requiring moderation.

Needless to say it was a spammer.

Can anyone shed any light on what might have gone wrong here?
