Field: Contact Us Options

Currently, a user can select "Other" and enter text in a box next to it, in addition to entering text in the "Message" box.
I would like to add another subject like the "Other" option in my Contact Us page.

How would I do this?
Is it as simple as copying and pasting the "other option" php language in the sendmessage.php file?
Does vBulletin provide support for this and what is the cost?
