Electronic Form Approval: HTML Code to do it
 

I need assistance in setting up an online form (within vB or vBa) that allows an end-user to mouse-click on a "box" with the "I agree to abide....bla bla bla." In in a nutshell* this electronic form setup would serve to legally validate the person's acceptance of my Advertising Agreement (or any other agreement for that matter).

I'm basically looking for the same type of setup as when a new member signs up to become a member of the discussion board. Like this: http://www.vbulletin.org/forum/attac...6&d=1173724544 but setup with my Advertising Agreement in place of the "Forum Rules" statement.

The reason for requiring such an online form is that the advertiser may not necessarily want to become a member of the board* but wants to advertise on it. This is the reason I have not incorporated the verbiage of the Advertising Agreement directly into the existing forum rules registration template.

Any suggestions for this code would be greatly appreciated* as this is holding me up from going live and I'm pulling my hair out to find something which will work for vB 3.8.4!!!!
