Problem with ads module

I have installed phpws 1.6.3. I attempted to create an advertiser and add a banner. I get this error when trying to upload a .jpg banner.

Missing folder information. Unacceptable file type.

Can anyone tell me what to look for and why it will not let me upload the banner?
