Standard form for a thread to ad music specifications
 

Is there any way i can have a standard form on making a new thread. What i am trying to this is this:
I am adding music names in the title. In the thread itself i want to give information about the song sutch as release date, label, style of the song and so on..

I was thinmking to have something there we have multiple select boxes and drop down menu and stuff like that..

Hope someone can point me in the right direction whit this.
