Favicon changed on its own
 

My favicon has changed to some red box with the letters BF in it. (Mine is pink with letters GP)

I have done all the steps in all the posts regarding the favicon and its not fixing it. When I check the url to it, its showing the correct one on the page but still in the address bar is the wrong one.

Any suggestion please? :)

I also have seen where several peoples have changed on their own and no one has explained why this happens.
