How do i do this? (First step of registration user id field)
 

Hi,

I am creating a page where I want something like this:

"To start of just enter your desired user id - [FIELD BOX HERE]"
Then there will be a continue registration button.

How do i get that box and then on clicking the button the guest goes to fill the other registration details.
