Forum Additional Registration options and Forum Rules are gone
 

I've noticed that my registration page looks like this:

It's missing my additional custom fields and the box that shows our forum rules. The default theme does not experience this issue, but I have no idea why they don't appear on my themes. Is there a certain template this applies to that I can reset?
