BB Code Small, specific improvements to the ACP bbcode manager

Please add the following to the ACP bbcode manager:

1. Allow disabling custom bbcodes. Currently the only option is to delete them.
2. Allow disabling the image/clickable button in the editor. Currently the only option is to blank the image field, but that's inconvenient because to re-enable the image you have to go look where you had placed it on your server.
3. Allow defining the order that the custom bbcode buttons appear in the editor. Apparently they are in the order that you create them.
4. Allow defining custom title text for the custom bbcode editor button. "Wrap X tags around selected text" is not always very descriptive or even accurate. For example, a [hr][/hr] horizontal line bbcode should say "Insert a horizontal line", not "Wrap [HR] tags around selected text".
5. Order the custom bbcodes in the bbcode manager. Currently, they appear to be unordered or based on the order of creation, instead of being alphabetical.
